"Intent To Apply Forms" for the focus area “Total Health & Well-Being.” 

Eligibility:

To determine if you are eligible for consideration during this funding cycle, please answer the questions below:

1. Does your organization have 501(c)(3) status, or do you have a fiscal agent that has 501(c)(3) status?

2. Does your organization serve the residents of Eau Claire County?  

3. Has your program been in existence for 3 or more years?

4. Does your program address one or more of the following outcomes: 

        - Reduce alcohol abuse by adults.

        - Reduce adult use of tobacco products.

        - Reduce the incidence of overweight and obesity in adults.

        - Increase recognition and treatment of adult depression.

        - Improve preventive dental care to, ultimately, reduce dental disease among low socio-economic populations.
